The Lystedt law concerning concussions and brain injuries in
youth sports became a law in July 2009. In compliance with
this law, all athletes and their parents must sign the PNS
Concussion Information Sheet that is attached. Pacific
Northwest Swimming just found out that this law applies to them
(previously they thought it did not), so that is why we are so late
in getting it out to everyone.
I will have copies of this at the Fall Picnic to hand out as
well. Please have your swimmer sign this form, have a parent
sign it, and please print your name clearly on the form as well, so
I will know whose form it is. You can leave the signed forms
in my bookkeeper file at Shoreline or Madison, or give them to me
at the Picnic. You can also mail them to me as well.
